Material record errors are corrected with visible context.

  1. Check the source

    A correction begins by reopening the cited clinic page and confirming the disputed field, unit, date, or wording.

  2. Repair the record

    The record and its check date are updated when a supported replacement is established. Edits that do not change meaning may be made without a formal note.

  3. Leave useful context

    A material correction states what changed and why when that context helps readers evaluate the record.
